What is this form for? We have travel insurance do we need one of these as well?
So used to going to America that I'd almost forgotten about it

Most EU countries have a reciprocal health agreement in that if you require minor treatment in a clinic or at a hospital you can produce your E111, sign a couple of forms and either receive the treatment free or for a nominal fee. The costs are then recovered through this agreement. This does not tend to cover much in the way of dental treatment, each country varies.

As I understand it, the rules have changed slightly and you must now obtain a different form for application, I think you now get a plastic card instead of a paper E111. The Post Office should be able to advise.

The E111 does not replace your Holiday Insurance, it should be used alongside, for minor treatments. It is a useful thing to have as we received free treatment in Spain when DS cut his foot badly on some cut glass, requiring stitches and daily dressings for the entire length of our holiday :rolleyes: All we had to do was produce our E111 each time we saw the doctor and sign a form for him.

It will not cover such things as operations, or repatriation in the event of serious illness or God forbid, death.

Diane's right - its a new form this yea rand you have to fill in a separate one for each member of the family - takes ages, but will avoid having to pay cash up front for any minor injuries.

The plastic card comes later - we got ours in the post just this week - but in the meantime the PO gives you a stamped paper form which covers you.

So you need to spend your Friday morning standing in the line at the post office, Rio - my advice - take a book (if your PO Qs are anything like ours....:D )

If you apply before Aug 31, you can still do it through the post office one form will do for all your family. after this date you have to apply by post, not sure where or what. So you are better of trying to get it done before date above as it is far easier.

our PO is usually ok, but it will be saturday as I work 8am till 6pm and they open and shut during these hours!!...lol.

Oh yes thanks.....:D ...it happened about five years ago. Though I think he's still smarting at the humiliation of having to 'drop his trousers' for the tetanus jab :eek: :D :D

So if I have the old form and not the new card, is it still valid?

The form is vaid until end of 2005, if issued after August 19th get the card automatically for 2006 but if issued before I think you have to reapply.

Warning:
Please ensure you have a revised E111, issued after 19 August 2004, for each member of your family. A revised E111 can be identified by it having an expiry date of 31 December 2005. After 31 December 2005, a European Health Insurance Card (EHIC) will be required.

and

Note: the European Health Insurance Card will replace the E111 in 2005. If you tick the appropriate box on the E111 application form you do not need to apply separately for this, as the details that you have provided will be used to issue you with one automatically later in the year, prior to the expiry of your E111.
