Is this form expected to be printed off and hand signed etc? What do I do with it? Just carry a copy?

Yes print it off and hand it in at check in with all your other documents re vaccinations and test results.

We just did the one on the Virgin web site when we did online check in - very simple form. They asked if we’d done it at the check in desk where they looked at vacc record and test, and that was it. Never mentioned again. At immigration he didn’t even want to look at anything other than passports even though the cabin crew said have them ready, they weren’t interested.

I had the same experience, really nothing to be concerned about - show the print out at check in and you're on your way :grin:

I didn’t print mine, I just submitted it to Virgin online. I did print my vaccine cert and negative test result and that was all immigration asked to see

Honestly folks have it all on your phones etc but paper copies too
My stuff all went through at check in in Glasgow ( we cans in via Antigua) on line and I don’t even tex DH didn’t so paper copies I’d done out
Then the same issue at Antigua to Florida for DH ( I won’t say I told you to have the paper bits tec works great when it works but I see issues with system over load
