19 year old MAY be covered if they are a full time student but not all policies include this so you need to check it first.

My advice would be to buy a group policy and not a family one for the four of you and then a separate one for him. Consider the following scenario:

Your sister is taken ill and it is serious enough that you have to cancel the holiday. Most insurance policies would allow for cancellation if a sibling was taken ill but not for an auntie. The four of you would have your holiday covered through insurance but not your son because the relationship isn't considered close enough.

It's probably worth giving an insurance company a call and see what they say, and what the difference in cost would be. They would be able to advise you best to make sure that you are fully covered.
