From the clues you have given about your age (40 years with husband etc), then I suspect that surgery is less likely to be helpful (rotator cuff dysfunction is very common, and even if there is a tear, it is unlikely to be able to be repaired), and physio ought to be the first step in any case. It might also be worth talking to your rheumatologist about this.
