Middle names are essentially ignored. Mine is on passport but not on booking, esta or api. As long as first name and surname match then that's all fine.

They are not necessary but if you use them make sure you perfectly match the passport to save any embarrassing questioning at the airport
Best just to skip using them

I put my middle name on there for international travel but my names are so long they never seem to fit it all on the ticket and just shrink my middle name from Angelina to Ange lol. Never had a problem though.

Best to book in the name as it appears on your passport the. It's less likely there'll be a problem

Yeah they do that to my name too, often missing a couple of letters from my name!
I actually do put down my full name (first, middle, last) on bookings. But I respond to my middle name so it only makes sense to do this (I've been asked at check in to verify my name before, and respond with my middle name - they get confused for a while, then see it on my passport/tickets/etc, and are happy).

My name on passport is Kirsty - Anne. I never have used the - Anne part and have never had any problems at all.
