Currently for outbound...

Proof of vaccination (I uploaded PDF from NHS)
Proof of negative test (again, uploaded PDF)
Attestation (signed digitally within the app)

Requirements have changed since I returned, but it was...

Proof of vaccination (same PDF as before)
Proof of completed passenger locator form (can't remember if I did just screenshot of QR code or uploaded entire PDF)

and presumably now proof of negative test, same as outbound

We used it and it saved us lots of time at check in.

Wish we could have used it on the way back.

The app makes everything so much quicker so use it if you can , I set it up on my phone and added the rest of my family as companions .

its very user friendly.
