it doesn't it says a relative can't do it so I would do it yourself or ask the social worker if you have one
i'm going to do this for my kids when I get them their first adult ones just after we get back from our summer holiday if anything comes back i will get the social worker to scribble something for me Edited at 09:54 AM. Reason: amend to correct the info |

I know it doesn't say that, that is my point. I have always done a covering letter explaining why I have signed etc. The passport office have told me, when I rang on Monday, that I need to get an additional letter to confirm she really is autistic and learning disabled. I asked why this information wasn't listed as an essential item for renewing her passport and she couldn't answer me.

I would write the letter and for good measure include a photocopy of one of your original diagnostic letters just to back up what you are saying and this should cover all bases
I doubt very much your application will be returned, it does sound like the person at the other end of the phone in the passport office was unsure themselves, you could probably phone again get someone more experienced and they would tell you exactly what it says on the website |
