Sworn statement for travelling to France

Hello we go to DLP on Friday on the Eurostar. I've just seen on the website we need to fill in this form.

Where it say "residing at" I'm guessing they want the hotel address not our residential home address?

We put home address. They didn’t really check it... just glanced to make sure we had one!
Also had to hand write as our printer was broken, but they were giving forms out at the station.

Hi there is this form just for those travelling by Eurostar? I have some Affidavit to complete which is shorter than the form you have. We’re flying with EasyJet on Monday.

We are supposed to be going on Tuesday if we stay off the quarantine list! I haven’t seen this form. I printed some other ones out to fill out . Not sure if these are new. I haven’t got a printer. I printed out the forms at work before I finished for two weeks last week. I may have to hand write them.

This was the one that was on the Eurostar website. Can't believe we go tomorrow and just hope we stay off the quarantine list.

We used the handwritten one. We took the printed ones off them just in case but didn’t fill them in.

What do you put as Final destination when staying on site? Its technically not Paris but is that close enough? Is Coupvray a better answer? Thats a region not a city though.
