We have!

First thing to be aware of is not all companies allow cross border travel -Hertz did when we did it but Discount don't! You need to tell them you are crossing!

We went from Vancouver Island via ferry to Seattle and then drove back into Canada. Totally doable! Just be prepared for a lot of questions! Where you are staying, how long etc!

We did it last year (with Hertz) and are doing it again this year (with Avis). I confirmed with both companies before booking that taking it over the border is allowed. They both said it is and there is no cost involved. We went from Canada to USA.

At the border you technically don't need an ESTA, although we had one anyway. We had to get out of our car and go into the processing room. There were quite a few people in there but we were called really quickly (less than 5 minutes). They asked us if we were on ESTA, and we said yes, but we still had to pay a few $ each for a Visa which was stapled into our passports.

Coming back in to Canada was a bit more straightforward. A few questions and straight back in.

Have you thought about Amtrak Cascades from Seattle to Vancouver? We have just done this and then picked car up later on in the trip
