Apart from alcohol smell on his breath that may be noticeable by the IO. As he would be entering the USA he is then subject to their rules immediately. Now I don’t know if under 21s in the USA are allowed to consume any alcohol at all at that age, but it may be worth checking out.
I’m not saying don’t do it, in fact my sons have done it hen they were under 21, and had no problems.

Slightly off topic, but I’m genuinely interested in people’s responses - people who choose to drink on planes, is it because it’s free that you want to drink, would you drink if you had to pay for it?

I don’t drink very much, normally only if I’m out with friends, and I don’t drink on flights as I guess it just doesn’t occur to me to have a drink.

For me this wouldn’t change whether the drinks were free or not so I’m just curious as to others reasons for having a drink.

Is it more of a relaxation thing? So if you were going to sit and watch a film at home would you have a drink? So the same whilst watching a film on a plane?

I have no problem with people drinking on flights btw, I’m just interested.

I usually have one G&T and a glass of wine with my meal, I would have this if I was paying or not but I do enjoy the fact that it’s included 😀 I just feel it’s a nice relaxing start to the holiday.

I'm pleased you asked this as it's something I often want to ask when reading posts about drinking on flights. I have the welcome drink before take off but that's it - I only drink soft drinks during the flight. I'm nervous on planes but alcohol doesn't relax me - it has the opposite effect in fact.

I:like to have a g and t on the plane and the wine with my meal. I would have it even if I had to pay. I like a drink and I’m on my holidays, so it’s something I would do. I don’t think drinking on planes should be banned, but I do think if necessary people should be refused drink before they got out of control. I was on a flight to Spain with my toddler son once and a large group of people were very drunk and loud and aggressive. I was on my own, as I was going to visit a friend. Luckily for me a very kind gentleman offered to escort us on the airport transit bus, he was shocked at the language these people were using in front of small children, it was awful.

I like a glass of wine with my meal on the plane, and I would pay. It’s part of the holiday. Oddly, I rarely have a drink at home, the next time will probably be Christmas.

It's more that it signals the start of your holiday.

If I was travelling for business purposes (and on train for that matter) I would not have a drink, not even a glass of wine with my meal (and I do enjoy a glass of wine).

But as you're on holiday, I tend to have a beer at the airport and then a drink or two on the plane.

I think this advise needs to be listened to carefully. As said, he can drink, but if stopped after landing, he could be refused entry if breathalysed. Is it worth the risk for a few drinks.
Worse would be if something happened during the short time after landing while in or out of the airport. If he fell over, acted weird, argued etc and security stopped him and found alcohol in his system. Being a minor, not only himself but you as parents would be charged. You would never be entitled to an esta again.
Extreme, but happens. Is it worth it for a drink.
Flying, tiredness, alcohol and the heat on arrival really arent a good mix.

Throw caution to the wind

Just book assistance when you land so they will help him off the plane onto a wheelchair. Immigration probably wouldnt question it too much.

Make sure its champagne though - travel in style
