BA’s policy is not to do it, but nobody is capable of the 100% you mention. I’ve seen it happen more than once on BA flights I took, it is then left to the crew to try and sort it out onboard.
Also read a very recent thread on this very subject on another forum about a BA flight not taking off until a 2 year old could be seated close enough to a parent to satisfy the CAA guidelines. It is much rarer on BA than other airlines and I’m not aware of any flights that actually took off without it being sorted, but BA doesn’t always manage it in advance. It can be slightly more likely in WT+ and CW, due to the smaller number of seats and the BA policy of charging many passengers for advance seat selection in these cabins.
