Sorry, another question about disabled parking permit.
I've received my email and I want to apply for my parents permit but what parts of the form do I fill? It is also asking for a doctors note, do I need to get this? I have my copies of the front and back of the blue badge and the passport.

It was the 83039 form I filled in. I completed the top section and clicked "temporary permit" then put the information. I then selected the disability type and that was it. I sent it along with a scan of my blue badge and my passport. Hope this helps.

Just fill in the top bit with travel dates. You dont need doctors note.
