I didn't realise there had been others Mr Tom, it's a pretty weird thing to do, hopefully not happened too often.

Surprisingly easy to do (although I've not done it!). Each vial has about 0.3ml of vaccine. You add 1.8ml saline water to dilute and then draw up 6 lots of 0.3ml. In a dizzy moment you could forget to add the water and draw up the usual 0.3ml and deliver!

Yup that would do it. Not done it myself either. We always take 6 syringes at the same time. That way you know how many you have done and how many to do.
