Get some flight socks. Regular laps of the plane and look up some DVT exercises to do in your seat.

I've had constantly painful legs/feet for years due to lymphoedema. I take Naproxen and Cocodamol to help keep the pain at bay. My trip to Orlando last year was my first flight for over 20 years (not just first long haul flight but first flight full stop) and I managed fine. I made sure I was dosed up on my meds and got up and walked around every so often.
