We did this in March because DD - pregnant- forgot to renew her EH1C. She did it online and she printed out the confirmation email and took that with her but luckily we didn’t need to test the system.

Thank you for all your responses very helpful. I called the EHIC helpline up first thing this morning was told I’d be sent a link. By lunchtime nothing came so I called back this afternoon. Spoke to a very helpful gentleman who explained everything and did the application over the phone with each of us. He then sent a link with what to do should we need assistance and the cards don’t arrive 👍🏻.

If there is no deal between the UK and the EU, reciprocal healthcare provision under the EHIC system may not longer be in place in EU member states. It would be just like travelling to any of the many other countries in the world where no reciprocal healthcare provision is available. You have insurance or you are on your own

Even if there is a deal, extension (in whatever format) of the arrangements would need to be part of that deal.
