I'd say that was a result. Keep the paper trail and speak to your legal man just in case a constructive dismissal thing comes in down the line.

Oh and lock your Google account down and put 2 factor authentication on it so that she can't possibly get back in.

Then you can just give her notice and terminate her employment. You don't need to give a reason. She doesn't have security of employment and you don't need to go down the gross misconduct route for unauthorised absence.
As for leave letter of the law is statutory holidays have to be given , when they can be taken is up to the employer . Theoretically an employer could just give employees dates and say "that's when your holidays are" in the real world this rarely happens apart from summer/Christmas factory shutdowns but its legal.

How any PA could think it was reasonable to go into your files to see what you had on them is ridiculous though. You are definitely well rid !

In our contracts we say words to the effect, your holiday entitlement is x amount per year. Please complete a holiday form and get it signed off before booking any holidays. We ask for 3 weeks notice of holidays.

Our formal disaplinary policy says we encourage employees to bring someone to the meeting for emotional support/to ask questions for them or to take notes for them. If this person is not an employee of the business we would require them to sign a confidentiality agreement to protect all parties.

No she didn't come into a weeks worth of work but did have some work from during the week. I had gave her minimal things to do with a aim to build up over time. In 5 hours she was doing about 1/2 hours worth of work. I honestly expected her to prioritise her work as I trained her to do.

At her interview I asked about holidays and she didn't mention this one.

We run our business with the same ethos as Google. We encourage/support/to help creativity in a laid back environment. The Google ethos works for our business as it reflects our beliefs.

But your laid back environment has caused you problems, no holiday form and then later you have said all the other work she has missed. Anyhow she has left, lucky for you 😉
