Help for a novice

Hi, hoping for some guidance, have only ever hired a car once as part of the package holiday in Florida. We are flying into and out of the same airport (Pittsburg) We only need a fiesta size car, for one week. I have so many questions I hope to get some advice on:

Which companies are most reliable?
Should we book through the airline?
What is best regarding the extra insurance? pay via the car hire company or buy the insurance here? (I've read buying it here you have to pay out then claim back? )
If buy the insurance here where from?
When is it best to get car hire? We don't travel for a few months yet (not long booked flights).

Is there anything else I should be considering?

Alamo brits / auto Europe/ hertz you need to book through a uk website with all the insurance included in the price. I personally like alamo as you can choose your car .

I have looked at alamo, £400 for the 10 days I need 😱
I looked on Expedia and the cheapest option there is dollar but still £350

I have just dummy rebooked flights and included car hire and it an extra £140 to include car hire. Anyone know how I can contact ba to add car hire to my booking?

They all seem to include collision damage waiver. Though.
