As someone getting too close to that birthday I would say that unless you really know your friends’s taste very well, steer away from jewellery and personalised Not on the highstreet stuff. Everyone has their own taste and for me - it’s too much and I like to choose my own keepsakes, jewellery and ornaments (minimal).
So I would go with the experience idea - it doesn’t need to be extravagant - theatre tix, a nice meal, coffee and cake, a trip to the zoo, a train to a favourite place from your joint past...
Spending time doing something is much more personal than a personalised object IYKWIM.

My favourite gift to receive (actually for my 40th) was a Photo Album with photos relevant to my life so far - including family members who are no longer around, my son as a baby etc. I loved it - but it must have taken ages to do as at my age many of the photos were on slides, or needed scanning in. You could ask around family and friends and ask them to hand write you a "memory" on a postcard and include that in the album too (I did that for my grandmother's 80th and she really loved that).
