Hi, I’ve just received this form in the post. Does anyone know what you put in the box foreign tax identifying number? Is it my tax code for the UK.
As a previous poster has said there are no instructions with the form even though each question says see instructions!

Sorry I can’t remember but I know I googled it and found a post with a picture of it completed for those living outside the UK.

Just had a search as I knew I kept a copy of mine.
I completed Part I with my name and address, the boxes for Country of Citizenship and Country must be your Country in full no abbreviations, so just st putting UK isn’t acceptable. Then dob in American format. Part II - again United Kingdom, then on down to sign, print name and date as mm-dd-yy. I left the box blank that was looking for a foreign tax identifying number. This was in November 2017, I sent it to them from AKL whilst there and I never heard another thing from them. Hope this helps.

I think we were told by the guy at the DVC stand at AKL that they are required to have this on the file.

I called member services about mine, as that's who sends it. The accounting department were able to advise on how to complete it, then I faxed it through to them.
