Hi - No you don't need a doctors signature. They ask you to send a copy of your Blue Badge I send both sides and a copy of the photo page of your passport,

Does anyone know if you have to pay ever year now, as I'm sure in the past couple you only paid every other year (so each placard effectively lasted for 2 summers).
Am happy to pay, since a small folk say, it's a minimal cost. However, it would be $17 towards a coffee when we're in Miami 😂

I told them when my trip was, Aug/Sep ,and they provided a placard which expires in Sept. I'm sure on the form it says 6 months max for a temp placard.

I believe I am right in saying that if you apply more than once in any 12 month period you don't have to pay again. But its worth double checking with them.

Ah, that makes sense Jennywren, knew I'd find the answer on here!
I thought I'd applied earlier and only realised yesterday that I hadn't so think I've been 5 days too late from applying last year (if that makes sense!)

Hi Jen - yes, you're correct. As per Florida Highway Safety,HERE

Temporary – Persons with short-term mobility impairments may receive a temporary disabled parking permit for up to six months as specified by a certifying medical authority. Temporary disabled person parking permits require a $15 fee. If the need for the temporary disabled person parking permit exceeds six months, an additional temporary permit may be issued. If the additional permit is applied for within 12 months from the issuance of the first permit, no additional fee is required.

So, we just got one today for my hubby. Really straightforward.
Go to tax office
Take passport, blue badge and proof of where you are staying
Pay $15

Temporary pass given there and then that lasts for 3 months.
