Is it possible to have an appetiser and main on the DDP table service meals instead of main and dessert

No, unfortunately not. You can ask restaurants if there are any substitutions available and they may offer soup, a house salad or fruit plate, but you can't just choose an appetiser from the menu. It's also at the discretion of the restaurant. If you want the be, you'd have to pay for it.

Nope. I think a lot of people would prefer that option, but desserts are cheaper than appetisers.
