How get copy of your hospital records

Hi does anyone know how get copy of my hospital records
Not after my gp ones just hospital ones
Would it just be ok to phone consultants office and ask or do I need put in writing

You would have to put it in writing. I know our local healthboard has a form that you can print off and fill in but they do charge for them.

You need to put in writing, your PALS dept at your hospital will be able to help with the contact details. You can get copies of clinic letters from the consultant secretary or GP though.

Search the hospitals website for access to records. The fee varies

Echoing above, you will need to put it in writing, it can also take some time as it is not a simple case of locating your notes and handing them over, they will have to be looked over and any third party references taken out.

I see you are in Edinburgh, the records are all held electronically there I believe. You should submit a request in writing to the Health Records dept at the hospital. They will print off the relevant info and will charge a fee for this. A few years ago I think it was about £10 but no idea what the cost is now.

I only need stuff for last 18months I've only been this consultant and that dept for that long anyway
