All the major car hire companies (inc. Avis) are in the car rental centre - a multi story block just outside the airport - however, you get to the centre via a tram (free) that runs from inside the airport - a 5 min journey.. We used Avis in July - picking up and returning the car was exceptionally easy! If you join Avis Preferred - you can go direct to the floor of the centre that Avis keep their cars - show your paperwork to the attendant in the cabin in the middle - collect your keys ... and go. Its as easy as that

I think it depends on whose driving and what your kids are like. I did it and did the drive straight away. For me it was easy and I don't sleep on the plane and the kids both slept for most of the drive. We stopped half way at some services and really enjoyed the experience. For me I wanted to be in Orlando ready to go the next morning so it was great but everybody is different.

The airport itself at MIA is still an absolute hovel but the terminal AA use is a bit better than some of the rest of it at least. The new rental car depot is basically onsite which is much better than the old ones which were in a very dodgy area, and it spits you out onto US27 which makes the drive easy.

If you're on the early flight from LHR you'll arrive into MIA by 2pm-ish which is generally a quieter time for immigration. I'd say you'd be able to be on the road by 4 so up in Orlando by 730/8ish...
