How strict is the ADR cancellation fee?

It's any of them that you book through the Disney site. Whether you're on the DP or paying OOP does not matter.

We rescheduled for a much later date and then cancelled the booking. We didn’t get charged for it :-)

Call guest services and explain. Tell them you had to cancel due to illness and they'll probably let it slide. We did exactly this in August this year. We cancelled Fantasmic Dining at Mama Melrose 12 hours in advance and explained that one of the party was unwell. They mentioned that it should have been 48 hours cancellation but as it was for illness then they would credit the DP back to the room.

They'll do their best to help but having a good excuse is probably a good plan.

managed to miss a couple on our last trip and never got charged, not foolproof but might not be the end of the world, we cancelled one within an hour of the reservation time due to child being ill and tired.

Yes, for all Disney restaurants - even with a DDP you have to link a card to the booking and will / should be charged $10pp if you don't cancel within 24 hours.

They charge $10 per person if noone attends. But if one person from your group shows for the reservation then there should be no charge.
