Fair enough - I take it back. It’s only select flights and ours (MAN-MCO) isn’t one of them. I’ll report back on check in timescales tomorrow

Would someone be able to confirm that these are the current and correct attestation forms for both vaccinated and unvaccinated, just triple checking my documentation for tomorrow.

No, you use the same form for everyone. Your second link is the previous form for citizens. This is the form that is valid for travel from tomorrow:

All sorted, found a recovery certificate on the 3rd page of the Covid app.
Now just need to read up on the forms we need and do an ESTA |

Does the passenger just complete the attestation form or does a doctor need to sign, in particular it is this section and am concerned about
cleared for travel by a licensed healthcare provider or public health official. On behalf of ****** I attest that such person has recovered from COVID-19 in the last 3 months (90 days), or the time period specified in current CDC guidance, after having previously tested positive for SARS-CoV-2 and has been cleared for travel by a licensed healthcare provider or public health official. |

On the attestation form, you only need to use the "on behalf of" for people who aren't able to sign for themselves, e.g. children.
However the recovery document itself needs to be from an official source, i.e. NHS letter. This document is your clearance, rather than the attestation form. |
