I’ve just come on to ask the same question. I’ve printed the form to fill in and I’ve done the attention with virgin online.
I wasn’t able to do anything online to fill out the form with CDC so presuming like already said it’s just print off and take with us!

For Virgin, there is nothing really to print off other than the front page that has a summary of all passengers and whether they have submitted their attestation.

Who are you flying with? Lots of help if it's Virgin?

If it's BA though just upload the Verifly app.

Enter main details then add a companion and fill in both personal details.

Then as you get them upload the NHS passport, negative tests and finally the attestation. Mine says I'm good to go.

Naturally as an old cynic I've printed them as well

Just to add I found verify easier than I i initially thought ,photo of yourself info ect and right near the very end you just slide the bar to yes And no questions for attestation and then your good to go! I did print everything out as well. We travel to Heathrow today so will see how easy verify Is for checking in.

Sorry I can't help - our daughters are in their forties.

Doesn't the Verifly app explain it? I just loaded myself and Joan as a companion. Maybe you just add children as companions?

Did you print it off as you were doing the attestation or do virgin have a print it off in their format somewhere?
