Hi Diz, as BB said you'll likely be fine as you're so close to going and the name update has yet to be enforced. You're heading off around the same time as my brother/his family, 21st July. They've ESTA's from early 2018 and are not reapplying. Have a smashing holiday
The US placed a 'sample pp' diagram next to the name question, even tho quite obvious many fail to see and are completing as per the question of name. Previously one only saw the update if they hovered over the ?.

Thanks I cannot see us being turned away as our first and family names match our passport but I agree if I had known to include middle name I would have although DS has 2 not sure how that would have worked. Fingers crossed all ok as we have been every year since 2009 and is our last visit for a couple of years due to puppy coming after this years holiday
