This. It is because it's a marriage document and not a change of name document.

A lot do not change their name. You don't have to.

Esta = passport

Insurance company will confirm their view but I'd go for name on passport as that's who is traveling.

It just annoys me that some think if you get married your name automatically changes. Your name is what your born with until you choose to change it.
