I am in the same situation. Holiday booked in maiden name to match my passport, my covid passport is in my married name. I went to my gp on fri to change my name back to my maiden name. I am still waiting on my covid passport updating. I called my gp today and they said it takes around a week to update

My doctor wanted sight of my passport and certificate and reverted my name. It was easy and quick.

My doctors just changed it, as I was reverting back to a name they had on the system already. They just switched it back

I’d only changed mine with them two years ago but they still wanted sight of the documents (actually emailed photographs of the docs) as (reassuringly and in line with DPA) they don’t retain copies.
