It's just underneath the top box. There is a check box for a temporary tag and a space to put your dates in.

I rang up last Tuesday or Wednesday to pay for mine and it arrived this morning.

The Orange County chap my brother dealt with advised... as a non-US citizen you need only complete the top section and where it asks for Florida driving licence or ID card simply write NON-US.

They say 60 days prior to travel.

you can send the forms in and they will tell you a date to phone up and pay--that's what they did with me

I signed it then scanned it to send back but not sure if you can do without.

😊 So used to doing it the old way glad of this thread! Question... How much of the form do we need to fill in? Just the top & tick temporary then just add front/back blue badge & copy of passport & email it? Want to send my DD's off now & then ring to pay at 60 days 😉 Thanks fellow dibbers, one less thing to worry about!
