We loved Tom Sawyer island too. Was a great place to escape to from the crowds - kids had a run around and explore and we sat and ate lunch in the covered over bit next to the barrel bridge. Just us and the squirrels! We also loved the riverboat, again a nice escape from the crowds.

I think the advice to plan everything once and then allow enough flexibility to redo favourites is very sound. Also with smaller kids I would go early and quit mid afternoon (quieter, much more chance of being able to re-ride without huge queues) or arrive late afternoon and stay for fireworks (need to Fastpass). We also found breaking up rides with the occasional show gave a chance to sit down and cool off for a bit. We used the touring plans site but flexed it to suit what we wanted.

Oh and the Buzz ride at HS is imho much much better for kids than the one at MK. You can meet Buzz and Woody directly opposite and the Green Army Men can sometimes be on patrol too and are a lot of fun.
