RIB and ID for Annual Pass

I've just bought 4 Annual Passes online and gone for the 11 months payments option. As part of the process, I'm now being asked to post the the following:

Mandat à dater, signer et renvoyer, accompagné :
• du RIB papier du compte bancaire ci-dessus,
• d'une copie de la pièce d'identité du titulaire du compte,
• l'engagement de paiement daté et signé

Any idea where I get a RIB (Relevé d’Identité Bancaire) from and do I just send a photocopy of my passport page as the "pièce d'identité du titulaire du compte"

Did you manage to sort it
I have just been looking and says you can only pay monthly if your in Europe as it says list of countries in the SEPA zone (in euro) i am trying to translate on google though if anyone can confirm you can pay monthly.
I have a non dated ticket i want to upgrade to annual pass but was told i can only do it at the gate.i wanted to sort before i went

Yep - got it sorted. I just sent a pdf copy of the front of one of my Bank Statements that had the IBAN and SWIFT codes on it along with a scanned copy of my passport photo. Emailed it all through and got the passes through the post same week.
